Concept Rehab in Sandusky, Ohio is seeking a Physical Therapist (PT) to join our team. We are open to Full-Time or Part-Time opportunities and value compassionate, dependable clinicians who deliver excellent patient care.

You will evaluate patients, develop individualized treatment plans, provide hands-on therapy, monitor progress, and educate patients and families. You will collaborate with nursing and therapy teams to improve mobility and quality of life.

Qualifications

• Current Ohio PT license.
• Graduate of an accredited PT program.
• Strong communication and interpersonal skills.
• Excellent clinical judgment and patient-care skills.
• New graduates and experienced PTs welcome to apply.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide hands-on physical therapy interventions.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with nursing, therapy, and interdisciplinary team members.
• Educate patients, families, and caregivers.
• Complete documentation accurately and efficiently.
• Help patients improve mobility, strength, function, and quality of life.
