• Assess patients’ abilities, medical history, and functional limitations to build individualized treatment plans.
• Provide hands-on therapy and therapeutic exercises, using modalities as appropriate.
• Educate patients and families on home exercises, safety, and recovery processes.
• Perform home safety assessments and recommend modifications to reduce fall risks.
• Maintain accurate patient records and report progress to the care team.
