Job summary

Liberty Senior Living is seeking an experienced Personal Care Assistant (PCA) to support residents with bathing, dressing, personal hygiene, and activities of daily living on each shift.

The role includes vital signs, record keeping, linen changes, and assisting CNAs. You will follow facility policies, maintain clean resident areas, and help with rehabilitation activities while ensuring prompt response to call bells.

Responsibilities

• Assists residents with bathing, dressing, personal hygiene and ADLs as needed during shift.
• Changes linen on beds with bath schedule and/or PRN.
• Takes and records vital signs per policy and/or as directed.
• Assists CNA with needed tasks.
• Turns residents while in bed per facility policy.
• Repositions residents while out of bed per facility policy.
• Assists with oral hygiene including denture care.
• Answers call bells promptly per policy.
• Completes patient care records at end of shift, including I&O.
• Fills in Patient Observation Records and Activity Attendance Records for the next month.
• Collects urine or stool specimens as directed.
• Keeps patient unit/halls clean and neat.
• Keeps bedside tables, chest of drawers, and closets clean and organized.
• Cares for and cleans nursing equipment (BP equipment, thermometers, etc.).
• Follows restraint policy, including release.
• Assists with post-mortem care when needed.
• Ensures residents have fresh ice water by refilling pitchers.
• Cleans water pitchers, cups, bedpans, urinals per schedules.
• Rinses personal clothing, sheets, etc. if soiled before washing.
• Assists residents with bedpan or urinal.
• Empties catheter bags and records output.
• Assists with rehabilitation programs and records on Rehabilitation sheet every shift.
• Reports to charge nurse with resident needs and treatments as directed.
• Keeps a clean towel and washcloth for each patient at all times.
• Empties incontinent pad barrel and replaces bags.
• Communicates end of shift to oncoming CNA’s.
• Understands and agrees to follow policy of no transfers until Certified, and to obtain assistance from a CNA when needed.
