Job summary

Liberty Healthcare and Rehabilitation Services is seeking a Personal Care Assistant (PCA) at The Foley Center at Chestnut Ridge. The role involves aiding residents with bathing, dressing, hygiene, and daily activities, while maintaining accurate records and adhering to facility policies.

Responsibilities include assisting CNAs, taking vital signs, supporting rehab activities, and ensuring clean, organized living spaces. Candidates must be CNA-ready and commit to ongoing training as required.

Qualifications

• Must be 18 years of age or older and have a high school diploma or equivalent.
• Graduate of a NA1 training program; must pass state test within 4 months from completion.
• Willing to care for geriatric patients and have a genuine concern for their welfare.
• Willing to follow orders and work well with other staff, families, and visitors.
• Attend in-service training and staff meetings as required.

Responsibilities

• Assist residents with bathing, dressing, personal hygiene and ADLs.
• Change bed linens on schedule and PRN as needed.
• Take and record vital signs per policy.
• Assist CNAs with tasks and support residents’ needs.
• Turn and reposition residents as required; maintain clean patient areas.
• Document care records at end of shift; complete observation/attendance records.
• Collect specimens and assist with rehabilitation activities; chart accordingly.
• Report to oncoming CNA and follow no-transfer policy until certified.
