Qualifications

• Ability to provide basic patient care including comfort, hygiene and vital signs.
• Assist with mobility and ensure safe transfers for independence.
• Report changes and concerns to the nurse promptly.
• Demonstrate professional communication with patients, families and staff.

Responsibilities

• Provide basic patient care including care and comfort, vital sign measurement, personal care and hygiene.
• Assist with mobility and perform transfer techniques to promote independence and safety.
• Act as liaison between patient and nurse to report changes and concerns.
• Provide high‑level customer service to patients, families, visitors, and employees.
• Document patient care and participate in hand‑off/report of pertinent information upon arrival and departure.
• Perform delegated tasks once competency has been validated.
• Individualize care based on patient interests and hobbies.
