Job summary

Qualifications

• Current State of Michigan CNA certificate preferred but not required.
• Minimum of one year of experience with frail or elderly population preferred.
• Ability to multi-task in team and independent settings.
• Valid driver's license and Michigan residency preferred.
• Experience with housekeeping, health practices, observation, reporting, and food handling preferred.
• Knowledge of HIPAA and PACE regulations.

Responsibilities

• Provide personal care services including toileting, bathing, eating assistance, grooming, and stand-by assist with ambulation.
• Perform two-person transfers and safe positioning using lifts and safeguards.
• Transport and escort participants to/from bus, center, activities, and appointments.
• Assist with feeding, showering, vital signs, and weight checks.
• Report unusual signs; communicate with other disciplines about participant conditions.
• Meal service, cleanup, maintain clean/ orderly care areas, assist during activities.
• Respond to alarms, monitor restraint checks, report mistreatment or property issues.
• Collaborate with departments to meet participant needs; maintain safe environment.
• Build trusting relationships with families/caregivers; provide emotional support.
