Qualifications

Responsibilities

• Provides basic patient care, comfort, personal hygiene and mobility assistance.
• Acts as liaison between patient and nurse to report changes and concerns.
• Provides high-level customer service to patients, families, visitors and staff.
• Documents patient care accurately and in a timely manner.
• Participates in hand-offs/reports of pertinent information during shift changes.
• Performs delegated tasks once competency is validated.
• Mobilizes patients using safe transfer techniques.
• Encourages independence and tailors care to patient interests and hobbies.
