Job summary

Qualifications

Responsibilities

• Assists nursing staff in delivering care and ensuring a clean, safe environment.
• Collects and records patient data, including vital signs and intake/output, reporting to RN/LPN.
• Aids patients with meals, bathing, grooming, and daily living activities.
• Assists with mobility and positioning using walkers, crutches, canes, or wheelchairs.
• Collects urine/stool samples and performs blood glucose monitoring per protocols.
• Communicates patient information to the care team and adheres to PPE and safety rules.
• Supports performance improvement and complies with safety standards.
