Hebrew SeniorLife Center in Dedham, MA is seeking a Patient Care Associate (PCA) to support residents with daily living activities and care plans, promoting comfort and quality of life.

You will join a compassionate team, assist with personal care, monitor vitals, document in the EMR, maintain a clean environment, and follow HIPAA and infection control guidelines.

CNA certification or active enrollment in nursing school is required/preferred; English proficiency and teamwork are essential.

Qualifications

• Long term care PCA certification preferred in good standing upon hire or current enrollment in nursing school.
• RSU PCA: CNA certification required in good standing upon hire and ongoing.
• Fluent English skills required – verbal and written.
• Computer literacy required.
• Flexibility and ability to work independently and collaboratively.

Responsibilities

• Recognize and respond to the needs of the patients and families.
• Recognize and communicate changes in the patient’s condition to the nursing team.
• Adhere to Safe Patient Handling practices and no manual lift environment.
• Participate in Quality Improvement programs.
• Assist patients with all aspects of personal care as indicated in care plans.
• Document pertinent information in the EMR.
• Report accidents and incidents as they occur.
• Observe and report skin integrity and patient abuse.
• Obtain vital signs and basic procedures as directed.
• Maintain infection control practices.
• Attend ongoing training and education classes.
• Maintain confidentiality and privacy per HIPAA.
