Job summary

Hebrew SeniorLife is seeking a Patient Care Associate in Dedham, MA to support daily living activities, patient comfort, and environmental care in a home-like setting. The PCA will be part of a caring team dedicated to high-quality care and patient dignity.

Role requires CNA certification or enrollment in nursing school, English fluency, and ability to work both independently and with a team. On-site duties include meal support, documentation, vital signs, and adherence to infection control

Qualifications

• Long term chronic care PCA CNA certification preferred in good standing upon hire or current enrollment in nursing school, with first clinical experience completed.
• RSU PCA: CNA certification required in good standing upon hire and ongoing.
• Fluent English skills required – verbal and written; computer literacy required; flexibility and ability to work independently and collaboratively; mandatory HSL/HRC educational requirements and programs.

Responsibilities

• Recognize and respond to the needs of patients and families
• Recognize and communicate changes in the patient's condition to the appropriate nursing team member
• Adhere to Safe Patient Handling practices (no manual lift)
• Participate in Quality Improvement programs
• Assist patients with all aspects of personal care as indicated in care plans
• Document pertinent information regarding care delivered in the designated areas of the patient's electronic medical record
• Report accidents and incidents as they occur
• Report observed or suspected patient abuse immediately
• Observe and report skin integrity
• Participate in daily report team and care planning meetings as needed
• Be proficient in obtaining vital signs and procedures (temperature, pulse, respirations, BP, weight, urine/stool samples)
• Follow infection control practices
• Attend ongoing training and educational classes
• Maintain confidentiality and privacy per HIPPA
• Communicate effectively with peers, other team members, patients and families
• Facilitate life enrichment activities that encourage patient engagement
• Maintain cleanliness of patient rooms and common areas
• Prepare, serve, and clean up meals and snacks
• Participate in committee meetings
• Other duties as assigned and trained to perform
