Responsibilities

• Implement day habilitation service plans (DHSP) for all members.
• Supervise and train members within the assigned program, and in community settings.
• Document progress and needs related to goals/objectives with data collection.
• Maintain a safe, professional, and respectful environment for members and families.
• Communicate effectively with members, peers, families and stakeholders.
• Support personal care, self-help, and independent living skills.
• Conduct crisis intervention and proper incident reporting when needed.
• Assist with transportation of members using agency vehicles.
