Job summary

Moments Hospice seeks a PRN Nurse Practitioner to deliver home-based palliative care under the supervision of a Medical Director. The NP evaluates patients, develops plans of care, and coordinates with families and healthcare teams to improve quality of life.

Qualifications

• Licensed NP with authority to practice in the state of Kansas.
• Master’s degree in nursing required and APRN certification (CHPN preferred).
• Strong communication and collaboration with interdisciplinary teams.

Responsibilities

• Provide palliative care consultations and manage symptoms.
• Perform histories, exams, and develop a plan of care with supervising physician.
• Prescribe medications and coordinate hospice care with the team.
• Educate patients and families about disease progression and care options.
• Maintain accurate documentation in medical records and HIPAA compliance.
