Job summary

Evolve Therapy Services, LLC in Hastings, PA seeks a Certified Occupational Therapy Assistant (COTA) for an in-house Skilled Nursing Facility on a PRN/part-time basis. You will support OT services and help deliver resident-centered care.

Responsibilities include implementing treatment plans, conducting screenings, coordinating schedules, documenting care and billing, and communicating evaluations to the care team. We offer competitive pay and growth opportunities based on status.

Qualifications

• Associate's degree in Occupational Therapy is required.
• Active OTA licensure in state(s) of practice is required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine the best intervention or treatment for each patient.
• Perform treatment plans and follow short- and long-term goals based on the plan of care.
• Interpret and communicate evaluation findings to residents, families/caregivers, and the interdisciplinary team.
• Establish individual resident therapy schedules with team members.
• Conduct therapy per the treatment plan and ensure proper documentation and billing of services.
• Perform other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point-of-care documentation.

Education
