Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant (COTA) for an in-house Skilled Nursing Facility in Humboldt, TN. This PRN position supports plan implementation and resident care under supervision.

You will conduct screenings, help develop treatment plans, document CMS-compliant notes, and collaborate with the care team to promote quality outcomes. Requires an OTA license and an associate degree; ongoing CE is encouraged.

Qualifications

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine the best intervention or treatment for each patient.
• Perform treatment plans for residents; follow short- and long-term goals and methods to achieve goals based on the plan of care by supervising therapist.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to the established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned and required to effectively treat patients.
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated.
