Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Johnstown, PA. You will assist in planning and delivering occupational therapy services to residents in a compliant, patient-centered setting.

Responsibilities include screening, implementing treatment plans, coordinating with an interdisciplinary team, and ensuring accurate CMS documentation and billing. Growth opportunities available.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention per scope of practice.
• Develop and follow short- and long-term goals per the plan of care.
• Communicate evaluation findings and recommendations to residents, families, and team.
• Establish resident therapy schedules with the team.
• Provide therapy per plan and ensure proper documentation and billing.
• Perform additional duties to effectively treat patients.
• Adhere to CMS documentation standards daily.
