Job summary

Visalia Post Acute in Visalia, CA is seeking a compassionate Occupational Therapist (OT) to join our rehab team. This full-time role offers $60.00–$65.00 per hour and involves evaluating residents, creating care plans, and communicating with physicians and families to promote independence in a skilled nursing setting.

The OT will supervise COTAs and students, ensure timely documentation, participate in care conferences, and support discharge planning and DME coordination.

Qualifications

• Bachelor’s degree in Occupational Therapy is required.
• Master’s or Doctorate in Occupational Therapy preferred.
• California OT license in good standing required and active.

Responsibilities

• Evaluate residents within 24 hours (or one business day) of referral.
• Develop individualized treatment plans and obtain physician approval.
• Supervise COTAs, aides, and students per state act and facility policy.
• Communicate progress with rehab teams and families; document timely notes.
• Complete evaluations, daily notes, and discharge documentation per rules.
