Oakwood Gardens Care Center in Fresno, CA is seeking a licensed Occupational Therapist (OT) for a full-time position with a sign-on bonus. Join a skilled nursing and rehab team focused on resident-centered care to help individuals regain independence.

You will evaluate residents, develop treatment plans, supervise COTAs, document progress, and participate in discharge planning while collaborating with nursing and therapy staff.

Qualifications

• Must hold a valid California Occupational Therapist license.
• Experience with EMR systems a plus (Casamba, PointClickCare).
• Skilled in delivering patient-centered therapy and collaborating with an interdisciplinary team.

Responsibilities

• Evaluate residents to determine functional abilities and therapy needs.
• Develop and implement individualized treatment plans.
• Provide direct therapy services per physician orders.
• Supervise COTAs and collaborate with the care team.
• Document progress and outcomes per standards.
• Participate in discharge planning and caregiver education.
