Job summary

Qualifications

• High school diploma or equivalent
• Basic nurse aide training or RN/LPN nursing modules preferred in community college
• One year acute care experience preferred

Responsibilities

• Provide patient care and observe patient needs, report to the Licensed Nurse
• Maintain proper body alignment for patients
• Assist with routine patient care per RN assessment and plan
• Hygiene, bed making, oral, peri and hair care, feeding assistance
• Assist with patient movement, ROM, turns every 2 hours, chair transfers
• Transport patients by gurney or wheel chair
• Vital signs including pain level
• Accurate intake and output documentation
• Hourly rounding to assess patient needs and safety
• Assist with elimination procedures and catheter/ostomy care
• Document in electronic medical record and ensure chart accuracy
