Job summary

Qualifications

• Must possess a minimum of a 10th grade education.
• Must be a licensed Certified Nursing Assistant in accordance with state law.
• CPR certification preferred.

Responsibilities

• Identify residents with wristband/photo ID before delivering care or meals.
• Provide routine daily nursing care per care plan and assessments.
• Record entries clearly on flow sheets, notes and charts.
• Report any changes in resident condition to Nurse Supervisor/Charge Nurse promptly.
• Assist with transferring residents between rooms or to/from vehicles as needed.
• Assist with daily activities and personal care, including feeding, bathing, dressing, and mobility.
• Maintain a clean, safe, and calm environment; observe for skin breakdowns and report accordingly.
• Protect resident information and adhere to privacy rules; report any unauthorized access.
