This role requires effective communication, teamwork, and physical ability to perform tasks such as ambulation, bathing, feeding, and vital signs documentation.

Qualifications

• High school diploma or GED is required (or as required by the organization).
• Ability to communicate effectively and work collaboratively as part of a healthcare team.
• Ability to perform the physical responsibilities associated with providing direct patient care.

Responsibilities

• Assist patients with daily living activities such as bathing, dressing, grooming, toileting, and mobility.
• Help patients transfer, reposition, and ambulate safely.
• Obtain and document vital signs, weight, intake/output, and other information as directed.
• Assist with meals and feeding according to each patient Care Plan.
• Respond promptly to patient needs and call lights.
• Maintain clean, safe, and comfortable patient rooms.
• Observe patients and promptly report changes in condition to the appropriate nurse.
• Assist nursing staff with patient care procedures within scope of practice.
• Follow infection prevention, safety, confidentiality, and facility policies.
• Communicate professionally with patients, families, and healthcare team members.
• Perform other patient-care duties as assigned.
