Qualifications

• Supports patients with activities of daily living.
• Documents in medical records accurately.
• Communicates health concerns to nurses.
• Assists with vital signs, intake/output, and hygiene.
• Follows written and oral instructions.

Responsibilities

• Assist patients with bathing, dressing, eating, and mobility.
• Take vital signs and document results in EMR.
• Support patients and families with care needs.
• Help reposition patients and transfer to wheelchairs.
• Report health concerns to licensed personnel.
