ProHealth Home Health and Hospice is seeking a Medical Social Worker (MSW) for PRN, part-time coverage in Bartow County, GA. The MSW will provide intermittent home-based social work services under physician orders and plan of care, supervised by the Director of Clinical Services/Nursing Supervisor.

The role requires a CSWE-accredited MSW degree (or related qualifications) and relevant healthcare experience; a valid Driver's License and MSW credentials are required.

Formación

• Master's or Doctoral degree in social work from a CSWE-accredited program.
• At least one year of social work experience in a health care setting.
• BSW with at least one year of health care social work experience (assistant).
• Two years of experience for assistant role.
• One year home care experience preferred.

Responsabilidades

• Understands and adheres to agency policies and procedures.
• Promotes the principles of QAPI in daily practice.
• Acts as a consultant to other agency personnel.
• Coordinates social services with community resources and partners.
• Maintains confidentiality and safeguards patient information.
• Documents services provided per agency policy.
• Assists in developing plan of care and coordinating social services.

Educación

Master's or Doctoral degree in Social Work (CSWE-accredited)

One year of social work experience in health care setting

BSW with at least one year health care experience (assistant)

Two years of appropriate experience (for assistant)

One year home care experience (preferred)
