Job summary

Heartland Home Health, a Gentiva company, is seeking a PRN/FLEX Social Worker (MSW) to provide emotional, social, and practical support for patients and families in home-based care within Michigan. You will be part of an interdisciplinary team focused on navigating illness, transitions, and care planning.

The role requires an MSW with Michigan licensure, at least 1 year in healthcare/home health or hospice, and strong communication, documentation, and empathetic skills to advocate for patients

Qualifications

• Master’s Degree in Social Work from a CSWE-accredited program.
• State licensure as required by the state of practice (Michigan).
• At least 1 year of social work experience in healthcare/home health or hospice.
• Deep understanding of psychosocial dynamics of illness, loss, and death; provide support during emotional times.
• Knowledge of hospice/home health services, Medicare/Medicaid guidelines, and care coordination.

Responsibilities

• Conduct psychosocial assessments and develop care goals aligned with the Plan of Care.
• Deliver emotional support, counseling, and crisis intervention to patients and caregivers.
• Educate families on community resources, advance directives, and care planning.
• Collaborate with nurses, therapists, physicians, and the care team for patient-centered care.
• Provide support through serious illness, loss, and end-of-life decisions.
• Communicate changes in patient/family conditions affecting care planning and safety.
• Document all services and interventions per regulatory and agency standards.
• Advocate for patients' dignity and rights with culturally competent care.
