Job summary

Amedisys Home Health in Georgia is seeking a PRN Medical Social Worker to support patients and families within the care plan. You will help address social, financial, and emotional needs and coordinate with the care team to ensure timely communication.

Requirements include a CSWE-accredited master's or doctoral degree in social work, at least one year in healthcare social work, CPR, and GA licensure eligibility. Per-visit pay is offered with flexible scheduling.

Qualifications

• Masters or Doctorate in Social Work from a CSWE-accredited program.
• One year of social work experience in a healthcare setting.
• Meets state and federal requirements for this position.
• Current CPR certification.

Responsibilities

• Assists patients and families with health-related financial, social and emotional concerns.
• Provides social work services per the plan of care.
• Communicates changes in clinical status to the healthcare team and care center staff.
• Educates patient and family on social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks in the patient environment and coordinates interventions.
