Qualifications

• Holds a master's or doctorate degree from a CSWE-accredited school.
• One year of social work experience in a healthcare setting preferred.
• Current CPR certification is required.

Responsibilities

• Assists patients and families with financial, social and emotional concerns; supports coping.
• Provides social work services per the plan of care.
• Communicates changes in patient status to the healthcare team promptly.
• Educates patient/family and team on social work interventions and plan of care.
• Performs care planning, transfer and discharge functions.
• Identifies risks in the patient’s environment and coordinates interventions.
• Other duties as assigned.
