Responsibilities

• Assist tenants with activities of daily living and follow service plans.
• Provide medication assistance under RN direction.
• Help with meals, laundry, and housekeeping per care plan.
• Monitor tenant health and safety and report changes to RN.
• Maintain cleanliness of residents' living spaces and common areas.
• Document changes and communicate with the care team.
