Job summary

Heritage Wellness & Rehabilitation is seeking a dependable Resident Transportation Driver to safely transport residents to and from medical appointments and healthcare facilities. You will work closely with nursing and clinical teams to ensure safety and comfort, while delivering excellent resident service.

The role emphasizes mobility assistance, documentation, and adherence to safety policies; CNA certification is preferred but not required, and BLS/CPR are advantageous.

Qualifications

• Valid driver’s license required.
• Clean driving record preferred.
• Experience in medical/resident transportation is a plus.
• CNA certification is preferred but not required.
• BLS/CPR certification preferred.
• Experience in SNF/long-term care is a plus.

Responsibilities

• Coordinate and schedule resident transportation based on appointment times and resident needs.
• Safely transport residents to and from doctor appointments, hospitals, and other healthcare facilities.
• Develop daily driving schedules based on resident appointments and transportation needs.
• Provide transportation for residents returning from hospital discharge.
• Assist residents with safe entry, exit, loading, and unloading.
• Assist clinical staff with lifting, transfers, and safe transportation of residents.
• Ensure resident privacy and safety during transport.
• Report changes in residents' condition to the nurse or supervisor.
