Belmont Village Oak Park is seeking a dedicated LVN/LPN to join our Assisted Living and Memory Care community. The role involves administering medications, supporting wellness initiatives, ensuring regulatory compliance, and guiding Caregivers and Medication Technicians in a fast-paced, hospitality-driven environment.

The position requires an active LVN/LPN license in Illinois, CPR certification (or ability to obtain within 30 days), and excellent English communication.

Qualifications

• Active LVN or LPN license in the state
• CPR certified or willing to obtain within 30 days
• One year of medication administration experience preferred
• Clear verbal and written English communication
• Intermediate computer and mobile device skills
• Professional, pleasant and team oriented attitude
• Consistent and reliable attendance essential for Belmont Village

Responsibilities

• Follow up on physicians orders, coordinate services and report changes to physicians and families
• Provide medication administration to residents as prescribed and document observations
• Ensure resident care and service plans are followed by supervising Medication Technicians and care providers
• Make appropriate situational decisions and manage conflicts positively
• Update and review resident medical records, clinical documentation and incident reports
