Job summary

Belmont Village Lincoln Park is seeking a part-time Licensed Practical Nurse for 1st and 2nd shift with weekend availability. You will administer medications, support wellness initiatives, ensure regulatory compliance and monitor quality in Assisted Living and Memory Care.

Lead and direct Caregivers and Medication Technicians, communicate with healthcare providers, families, and team members, and provide timely clinical interventions while maintaining exceptional hospitality for seniors.

Qualifications

• Active LVN or LPN license in the state of Illinois.
• CPR Certified or ability to obtain within 30 days of hire.
• One year of medication administration experience preferred.
• Clear verbal and written English communication.
• Intermediate computer and mobile device skills.
• Professional, pleasant and team oriented attitude.
• Consistent and reliable attendance is essential.

Responsibilities

• Follow up on physicians orders, coordinate services with ancillary providers and report changes to physicians and families.
• Provide medication administration to an assigned group of residents per physicians orders and document observations.
• Oversee Medication Technicians and care providers to ensure care plans are followed.
• Make situational decisions and manage conflict positively and constructively.
• Update and review resident medical records, clinical documentation, and incident reports.
