Job summary

Elara Caring is seeking a Licensed Vocational/Practical Nurse to join our home health team in Oklahoma. You will follow individualized care plans, monitor patients, and educate families to support health and safety in the home.

You will collaborate with the RN Case Manager, document medications, and communicate with physicians and families to ensure coordinated care. Travel between patient homes is required; prior hospice/home health experience is a plus.

Qualifications

• Current State License as a Licensed Vocational/Practical Nurse (LVN/LPN).
• Graduate of an accredited certificate, diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting.
• Ability to lift up to 50 pounds and assist with transfers or equipment.
• Valid driver’s license or reliable transportation to travel between patient homes.

Responsibilities

• Follow each patient’s care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Educate patients and caregivers on self-care techniques.
• Update care plans with the RN Case Manager as needs change.
• Document medication regimens and update profiles per guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations through proactive care.
• Deliver compassionate, high-quality care to support positive outcomes.
