Job summary

Elara Caring in Oklahoma is seeking a dedicated Licensed Practical Nurse to join our home health team. You will follow individualized care plans, monitor patients, and work with the RN Case Manager to adjust plans as needs change.

You will document medications, communicate with physicians and families, and help prevent hospitalizations through proactive, compassionate care. Eligibility includes a current LPN license, 1 year of clinical experience, and valid transportation to patient homes.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to lift, push, pull, and support up to 50 pounds
• Must possess a valid driver's license or have reliable transportation to travel between patient homes
• Experience in a hospice or home health environment preferred

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes
