Qualifications

• High school diploma or GED required.
• LPN license in the state of practice and residence is required.
• Current CPR certification.
• Minimum 1 year clinical experience; home health preferred.
• Minimum 2 years administrative experience in healthcare is preferred.
• Home health/hospice experience is a plus.
• Solid knowledge of home care framework and functions.
• Proficiency with Microsoft Office and industry software.

Responsibilities

• Greets visitors professionally and courteously.
• Answers incoming calls from patients, staff, and providers and routes them appropriately.
• Takes and relays messages promptly to the right person.
• Assigns clinicians to patients based on needs, location, and clinician skill.
• Maintains visit data in the electronic medical record system.
