Job summary

Visiting Nurse Association of Southeast Missouri seeks a Licensed Practical Nurse to deliver skilled hospice care in patients' homes and related facilities, following physician orders and CMS guidelines.

You will document observations in the EMR, participate in IDG meetings, manage medication reconciliation weekly, and handle on-call rotations, while communicating changes promptly to the RN, physician, and families as outlined in the care plan.

Qualifications

• Licensed practical nurse with hospice/home health experience and state licensure.
• Knowledge of CMS regulations and scope of LPN practice.
• Ability to document and communicate with RN, physician, and IDG team.
• Proficient in EMR documentation and medication reconciliation weekly.
• Willingness to participate in on-call rotation as scheduled.

Responsibilities

• Provide skilled nursing care in home hospice per orders and CMS guidelines.
• Take pertinent nursing observations and communicate with RN, TCM, and IDG.
• Attend IDG meetings and document notes.
• Maintain current clinical notes in EMR and update care plans.
• Reconcile medications weekly and inform doctor of changes.
• Assist RN in specialized duties and follow the care plan.
• Provide accurate information to patients and families per the plan.
• Follow state guidelines for LPN scope in homecare/hospice.
