Job summary

A Licensed Practical Nurse (LPN) is needed to provide individualized nursing care for elderly or acutely ill residents in a long-term care setting in Oregon. Under supervision of an RN, you will perform direct patient care, participate in care planning, and assist with medications and treatments.

Role requires current LPN license, CPR certification, and 6 months LTC experience preferred. Strong communication with residents, families, and the health care team is essential.

Qualifications

• Graduate of accredited LPN School of Nursing.
• Current valid LPN license in the state employed.
• Valid CPR certification.
• Six months experience in a long-term care environment preferred.

Responsibilities

• Provide direct care and assist with care as directed within scope of practice and competency.
• Participate in resident evaluations and care planning; coordinate care with RN and family.
• Review physician orders, document in the clinical record, and communicate with physicians.
• Assist with medication administration and maintain narcotic records as required.
• Maintain supplies and equipment on medication/treatment carts; prepare and document medications as prescribed.
• Support clinical discharge processes and assist consultants when needed.
