Job summary

Chestnut Woods Rehabilitation & Healthcare Center is seeking a Licensed Practical Nurse (LPN) in Massachusetts. Must have 1 year of experience. PRN opportunities and daily pay available. Shift differential up to $3.80 per hour.

$34-$42.80 per hour all-inclusive. Join a facility focused on compassionate, high-quality resident care and strong core values. Responsibilities include patient care per plan of care, medication administration, care-plan follow-up, and accurate documentation.

Qualifications

• Current, active LPN license in Massachusetts.
• Valid BLS (CPR) certification.
• Ability to care for multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide patient care in accordance with accepted nursing standards and the plan of care.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Administer medications and prescribed treatments per the plan and facility policies.
• Monitor residents, report changes promptly to supervising staff, and respond to clinical needs.
• Maintain accurate, timely documentation in line with orders and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and families.
