Job summary

The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse, advanced practice provider, or physician.

Ensures the health, comfort and safety of residents; administers medications or treatments, documents interactions and outcomes, and collaborates with health care professionals to support residents.

Qualifications

• Graduate from an accredited practical nursing program and hold an LPN license.
• Associate degree in practical nursing preferred.
• Current unencumbered LPN license with State Nursing board or multi-state licensure.
• Completes required annual CEU and competency training.
• IV therapy/management skills may be required in specific states.

Responsibilities

• Provides professional nursing care for residents in long-term care under supervision of a RN, APN, or physician.
• Documents resident interactions and outcomes; administers medications or treatments as ordered.
• Maintains confidentiality and effective communication within the interdisciplinary care team.
• Counsels residents and families on prevention and treatment plans.
