Job summary

Reliant Care Management, LLC is seeking an LPN to provide compassionate nursing care to residents. The role involves conducting assessments, documenting changes, and following established protocols in collaboration with the medical team.

You will participate in admissions, discharges, and wound care, while guiding CNAs and CMTs to maintain high standards of care. Emergency response readiness is required.

Qualifications

• LPN licensure or certification required.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications.
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
