Job summary

Wellpath is hiring a Licensed Practical Nurse (LPN) to provide direct patient care in a supervised setting. You will assess patients’ needs, administer medications including IV therapy if certified, and support treatment planning.

The role requires an accredited nursing program graduate and an active LPN license with CPR certification. Clinic experience is preferred, and the position emphasizes teamwork, continuous training, and high-quality patient care.

Qualifications

• Graduate from an accredited school of nursing.
• 1+ year clinic experience preferred.
• Current LPN/LVN license in state of employment; CPR certification required.

Responsibilities

• Under RN supervision, assess patients’ physical, psychological, and social dimensions.
• Administer medications and IV therapy where certified; perform diagnostic tests per protocol.
• Collaborate in controlled substance counts and respond to health emergencies.
• Assist with medical procedures, nutrition plans, and SOAP charting for accurate documentation.
• Attend mandatory staff meetings and ongoing professional development.
