Job summary

Visiting Nurse Health System is seeking a Licensed Practical Nurse (LPN) to join our Home Health team in the South Fulton area. This non-exempt role emphasizes compassionate, patient-centered care and adherence to VNHS values of Respect, Integrity, Excellence, and Stewardship.

You will provide nursing care under physician orders, educate patients and families, document care, and coordinate visits from our Hartsfield office near the airport.

Qualifications

• LPN graduate from an accredited program.
• Completion of IV training or IV therapy certification.
• Experience with advanced wound therapies is desirable.
• Current Georgia LPN license approved by the Georgia State Board of Nursing.
• 1–2 years in a Medicare-certified home health setting preferred.
• 2+ years as an LPN.

Responsibilities

• Provide nursing care, emotional support, patient and family education regarding disease process and self-care.
• Deliver nursing services as ordered by the physician and under RN direction.
• Report subjective and objective patient findings to the RN.
• Obtain physician orders under RN guidance and ensure orders exist for each visit.
• Document care rendered and changes in patient status to ensure continuity of care.
• Demonstrate infection control standards per VNHS policy and consistently apply them.
