Job summary

VitalCaring is seeking a Licensed Practical Nurse/LVN to provide hands-on home health care as part of a field-based care team. You will work under an RN, delivering treatments, monitoring patients, and reinforcing education in patients’ homes.

Qualifications

• Active LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum one year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus).
• Strong clinical and observational skills.
• Clear communication and patient education abilities.
• Ability to follow care plans while recognizing and reporting changes in condition.
• Basic proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide direct patient care under the supervision of the RN and implement the plan of care.
• Deliver skilled nursing services, including treatments, medication support, and wound care.
• Monitor patient conditions and report changes to the RN and care team.
• Educate patients and caregivers on self-care, treatments, and disease management.
• Maintain infection control and proper use of equipment and supplies.
• Document patient care accurately and timely within the EMR.
