Job summary

Visiting Nurse Health System (Visiting Nurse) is seeking a Licensed Practical Nurse (LPN) to join our Home Health team in Conyers and Covington. This role emphasizes compassionate patient care, education, and coordination with our clinical team.

You will provide nursing care and follow physician orders, with a Monday–Friday schedule. The position is headquartered at our Ravina corporate office in Dunwoody, GA, and offers competitive benefits and support for professional growth.

Qualifications

• LPN graduate from an accredited nursing program.
• IV therapy training or certification required.
• Experience with advanced wound therapies.
• Georgia State Board of Nursing licensure.
• 1–2 years in Medicare-certified home health (preferred).
• Minimum 2 years as an LPN.
• Valid driver’s license, auto insurance, and reliable vehicle.

Responsibilities

• Provide nursing care, emotional support, patient and family education regarding disease process, self-care techniques, prevention strategies, clinical nursing observations, and documentation for Home Health patients.
• Provides nursing services as ordered by the physician and under the direction of the RN.
• Reports subjective and objective patient findings to the RN.
• Under the direction of the RN, obtains physician's orders and ensures orders exist for each visit made.
• Provides necessary skills to report changes appropriately; documents pertinent information and care rendered to the patient to ensure continuity of care.
• Demonstrates knowledge of infection control standards and techniques as per VNHS policy and procedures and consistently practices these techniques.
