Job summary

Visiting Nurse Health System in Ellenwood, GA is seeking a licensed practical nurse (LPN) to join our Home Health team. The role involves providing nursing care, patient education, and coordinating care with RNs to ensure quality at-home visits.

This full-time, non-exempt position requires an active Georgia license, IV therapy training, and experience in home health. A reliable vehicle and a valid driver's license are also preferred to travel to Fayetteville and Jonesboro areas.

Qualifications

• LPN Graduate from an accredited school of nursing or program.
• Completion of an IV training program or certification in IV therapy.
• Experienced in advanced wound therapies.
• Current license by the Georgia State Board of Nursing.
• Minimum of one - two (1-2) years’ experience in a Medicare-certified home health agency is preferred.
• Minimum of two (2) years as an LPN.
• Current valid Driver’s License, proof of auto insurance, and a reliable vehicle.

Responsibilities

• Provide nursing care, emotional support, patient and family education regarding disease process, self-care techniques, prevention strategies, clinical nursing observations, and documentation for Home Health patients.
• Provides nursing services as ordered by the physician and under the direction of the RN.
• Reports subjective and objective patient findings to the RN.
• Under the direction of the RN, obtains physician’s orders and ensures orders exist for each visit made,
• Provides necessary skills to report changes appropriately; documents pertinent information and care rendered to the patient to ensure continuity of care.
• Demonstrates knowledge of infection control standards and techniques as per VNHS policy and procedures and consistently practices these techniques.
