Job summary

Elite Home Health seeks a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing care in patients' homes under RN direction. This PRN role covers Lake Panasoffkee and surrounding areas and supports Medicare-driven home health services.

The ideal candidate has an accredited LPN program background, Florida license, CPR, and reliable transportation. Prior home health or hospice experience is preferred. You will document in the EMR and collaborate with the care team.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current Florida LPN license in good standing.
• Minimum 1 year of nursing experience preferred.
• Home health, hospice, or acute care experience preferred.
• Current CPR classification.
• Valid driver's license, reliable transportation, and auto insurance.
• Ability to meet all required background and compliance requirements.

Responsibilities

• Provide skilled nursing care to patients in the home setting.
• Administer medications and treatments within LPN scope of practice.
• Perform wound care, dressing changes, catheter care, and other skilled tasks.
• Monitor patient condition and report changes to the supervising RN.
• Reinforce patient and caregiver education.
• Follow established plans of care and physician orders.
• Complete accurate and timely documentation in the EMR.
• Communicate with RNs, physicians, and other care team members.
• Participate in required training, meetings, and in-services.
