Pinnacle Home Care is hiring a Licensed Practical Nurse (LPN) PRN to provide in-home care across Florida. The role supports RN supervision, delivers skilled nursing services, and ensures documentation is accurate in the electronic medical record.

Ideal candidates have a Florida LPN/LVN license, one year of nursing experience, and a valid BLS. A driver’s license and reliable vehicle are required for daily home visits and flexible scheduling options are available.

Qualifications

• Must hold a current LPN/LVN license for the state of Florida.
• Graduate of an accredited nursing program.
• Valid BLS certification required.

Responsibilities

• Administer skilled nursing care under RN supervision.
• Perform curative, rehabilitative, or preventive care tasks per patient care plans.
• Complete visit reports promptly within the EMR system and ensure regulatory compliance.
• Coordinate daily visit schedules for seamless patient care.
• Support QAPI initiatives and participate in quality improvement efforts.
• Uphold patient rights and adhere to agency policies on discharge and documentation.
