Job summary

Elite Home Health is seeking a PRN Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes within the Leesburg area. Work under RN direction for a Medicare-certified home health agency committed to compassionate, high-quality care.

The ideal candidate holds a Florida LPN license, CPR, and at least 1 year of nursing experience, with home health or hospice background preferred. Competitive pay and growth opportunities available.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current Florida LPN license in good standing.
• Minimum 1 year of nursing experience preferred.
• Home health, hospice, or acute care experience preferred.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Ability to meet all required background and compliance requirements.

Responsibilities

• Provide skilled nursing care to patients in the home setting.
• Administer medications and treatments within LPN scope of practice.
• Perform wound care, dressing changes, catheter care, and other skilled nursing tasks.
• Monitor patient condition and report changes to the supervising RN.
• Reinforce patient and caregiver education.
• Complete accurate and timely documentation in the EMR.
• Communicate with RNs, physicians, and other members of the care team.
• Participate in required training, meetings, and in-services.
• Provide care in accordance with Medicare requirements, state regulations, and agency policies.

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes under the direction of a Registered Nurse (RN).

Position Summary

The LPN provides skilled nursing services in accordance with physician orders, established care plans, RN direction, agency policies, and applicable state and federal regulations.

Responsibilities

• Provide skilled nursing care to patients in the home setting
