Job summary
St. Elizabeth's Health at Home in Florence, KY seeks a compassionate LPN/LVN to provide direct patient care in a home health setting per approved treatment plans. You will collaborate with the RN Case Manager and clinical team to ensure quality outcomes.
Responsibilities include patient visits, timely documentation, patient education, and coordination with the physician. Requires active LPN/LVN license, CPR/BLS, and at least 1 year of nursing experience; driver’s license and home health software
Qualifications
- Graduate of an accredited practical nursing program and licensed to practice in the state.
- Current CPR or BLS certification with hands-on demonstration is required and maintained.
- Minimum of one year nursing experience; home care or hospital nursing preferred.
Responsibilities
- Deliver direct patient care based on approved treatment plans per protocols.
- Collaborate with the RN Case Manager and healthcare team for patient care.
- Document care, monitor patient conditions, and educate patients and caregivers.
- Complete and submit required documentation accurately and promptly.
