Job summary

At Home Healthcare in Lexington, KY is seeking a skilled Home Health Nurse to provide nursing care to patients in their homes, following the service plan and physician orders. You will perform all aspects of patient care, document progress notes, manage visit verifications, and collaborate with the Director of Patient Care Services to ensure quality, safety, and timely delivery of home healthcare services.

Qualifications

Valid Kentucky nursing license (LPN or RN) required.
Current CPR certification or willingness to obtain.
Current TB skin test or willingness to obtain.
Competency in general nursing skills, techniques, and practice.
Private duty experience helpful, but not required.

Responsibilities

Provides skilled nursing care in the home to patients per service plan, physicians orders, and agency policies.
Performs all aspects of patient care ensuring safety and reducing medical/health care errors.
Promotes and maintains patient health through teaching and rehabilitative measures.
Assists patients in learning appropriate self-care techniques.
Prepares clinical progress notes and follows the client service plan as developed by the RN and agency policy.
Submits all notes and visit verifications timely and efficiently.
