Job summary

Adoration Health is seeking a Home Health Licensed Practical Nurse to join our Mount Sterling, KY team. The LPN will deliver patient-centered, 1:1 home health care, administering medications and supplies while documenting care and communicating with families and the care team.

The role requires an LPN/LPT/LVN degree and an active license, CPR certification, and at least 1 year of nursing experience. Full-time schedule with diverse patient needs in a supportive environment.

Qualifications

• LPN/LPT/LVN degree with current unrestricted license in state of practice
• CPR certification required
• 1+ years nursing experience preferred
• Home health care experience preferred

Responsibilities

• Providing 1:1 care following the client’s plan of care at all times
• Administering medications, feedings, oxygen, ostomy care as needed
• Documenting nursing actions and communication with family, case manager, physicians
• Coordinating or ordering supplies, medications, and feedings as needed
• Ensuring all patient needs are met per physician instructions
