Job summary

Health at Home is seeking a dedicated LPN/LVN to provide skilled nursing care in patients' homes under RN supervision. You will perform assessments, administer treatments, educate families, and document care within regulatory standards.

Responsibilities include coordinating with the care team, supporting care planning, and staying current with nursing best practices and Medicare requirements. A valid license and reliable transportation are required.

Qualifications

• Active LPN/LVN license in state of practice.
• CPR certification current.
• 1 year of clinical home health or comparable experience preferred.
• Proficient with EMR documentation.
• Valid driver's license and reliable transportation.

Responsibilities

• Assist with data collection for skilled assessments under RN supervision.
• Schedule and complete nursing visits within agency timelines.
• Provide skilled nursing care including wound care, meds, catheter care, injections.
• Educate patients and families on medications, treatments, and self-care.
• Document accurately within required time frames and regulatory standards.
• Coordinate with the care team and refer to additional services when needed.
• Stay current with nursing best practices and quality initiatives.
