Qualifications

• Must have completed an approved practical nursing program or be licensed in the state.
• Minimum three years nursing experience preferred.
• Home health or medical/surgical experience preferred.

Responsibilities

• Provides direct patient care as defined in State’s Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation per the plan of care.
• Assesses and educates patient and family/caregiver about diagnosis and plan of care.
• Coordinates home health services and communicates needs to the RN or Director of Nursing.
• Uses equipment and supplies effectively and participates in growth and development.
• Performs other duties as assigned by the registered nurse.
